The Integrated Team Care (ITC) program supports Aboriginal and Torres Strait Islander people with chronic health conditions, by overcoming barriers between primary and secondary care, physical and mental health care, and health and social care so that they receive holistic care when they need it. Importantly, the program also provides health education and support to the medical community to deliver culturally appropriate care to Aboriginal and Torres Strait Islander people.
